Limited Waiting List Opening: ONLY for Adults with Disabilities Leaving Institutional Settings

If you or an adult in your household has a disability and is transitioning out of an institutional living setting, you may qualify for help paying rent.

Mainstream Vouchers are available to certain adults with disabilities. Mainstream Vouchers are different than Home Forward’s other Housing Choice Vouchers (Section 8).  We are ONLY opening our Housing Choice Voucher (Section 8) waitlist for individuals and families eligible for Home Forward’s Mainstream Voucher Waiting List Preference.

You may be eligible for Home Forward’s Mainstream Voucher waiting list preference if:

  • You or someone in your household has a documented disability

 and

  • The person with a documented disability is between the ages of 18 and 61 years

and

  • That person is transitioning out of an institutional or other segregated setting

One person in the applicant household must meet all three of the above criteria.

Examples of institutional or segregated settings may include, but are not limited to:

  • A group home or adult foster care for individuals with disabilities
  • A nursing facility, rehabilitation facility or other recuperative care facility
  • An inpatient treatment facility for mental health, substance use disorders, or other disabilities

How to Apply:

  1. Fill out all of the required forms. You can find them below. The forms include more details about how to determine if you’re eligible for this type of voucher.
  2. Submit the application and forms between 3/18 and 3/22 to Home Forward by mail, in person, or by fax:

If application and supporting materials are mailed, they must be postmarked by March 22nd, 2019.

If application and supporting materials are emailed or faxed, they must be received by March 22nd, 2019.

We will only be accepting applications and supporting documentation during the week the waiting list is open. However, applicants should begin collecting necessary documentation of eligibility prior to the opening so they are prepared to submit complete applications when the waiting list is open.

What happens next:

Once your eligibility has been verified you’ll be entered into a lottery. All eligible applications received by the deadline have an equal chance of getting a voucher (it’s not first come, first served). Home Forward will randomly select households eligible for Home Forward’s Mainstream Voucher Preference.  These randomly selected eligible households will be added to the Housing Choice Voucher waiting list. All applicants will be notified.  We estimate that all applicants will be notified in writing of their status by May 1st, but the timing of actual notifications will depend on the number of applications Home Forward receives.

I’m not eligible for a mainstream voucher, but I need housing. What should I do?

If you are already on our waitlist for a Housing Choice voucher (Section 8) you don’t need to do anything.  You can get updates on your position on the waitlist by calling the Waitlist Position Hotline at (503) 415-8000.
